Why Highmark denies Wegovy

Wegovy (semaglutide 2.4 mg) is FDA-approved for chronic weight management in adults with obesity (BMI 30 or higher), or with excess weight (BMI 27 or higher) plus a weight-related condition. Highmark can cover it under the pharmacy benefit, but it usually sits behind prior authorization and, on many plans, step therapy. Denials tend to follow a few patterns:

  • Not medically necessary. Highmark frequently applies a blanket 'not medically necessary' designation. A denial often means the submitted records did not, in Highmark's reading, meet the criteria in its medical policy, even when the prescription is appropriate.
